Reports must be submitted no later than NOVEMBER 1, 2023.

Please complete the report for all Kindergarten, 1st grade, 7th grade and 12th grade students enrolled in your school. All students (preschool and grades K-12) attending a public, private or parochial school in Idaho must meet minimum immunization requirements, be conditionally admitted, or have a valid exemption form on file to register and attend school (IDAPA 16.02.15).

School immunization reports must be submitted online. Reports that are faxed or mailed will not be accepted. If you do not have Internet access, please contact the Idaho Immunization Program as soon as possible to make alternate arrangements.

Immunization Requirements

To enter or transfer into a public or private school, all students in preschool and grades K-12 must meet immunization requirements outlined below at registration and before attendance. No child shall attend school without proof of immunization status.

  1. Preschool students need only be age-appropriately immunized with the required vaccines.
  2. DTaP: The 5th dose is not necessary if the 4th dose was administered at age 4 years or older and at least 6 months after previous dose.
  3. Polio: The 4th dose is not necessary if the 3rd dose was administered at age 4 years or older and at least 6 months after previous dose. For children born after 9/1/2005, at least one dose of polio should be given at age 4 years or older and at least 6 months after the previous dose.
  4. Varicella: History of chickenpox disease documented by a physician or licensed health care professional meets the requirement.
  5. Children should have one dose of meningococcal (Men ACWY) vaccine before the 7th grade. A second dose of Men ACWY is required prior to admission into the 12th grade. However, if a child received their first MenACWY vaccine at age 16 years or older, no additional MenACWY doses are needed for 12th grade admission.

Prior to completing the online report, we recommend that you print a paper copy of the School Immunization Report and use it as a worksheet. You will need your username and password to log in and complete your report. Reporting Checklist:

  • PRINT a paper copy of the School Immunization Report Worksheet and complete it before attempting to submit your report online.
  • Review immunization records of all students attending your school to ensure compliance with Idaho's school immunization requirements.
  • Complete the online school immunization report for Kindergarten, 1st grade, 7th grade and 12th grade students enrolled in your school no later than NOVEMBER 1, 2023.
